Please fill out if you are fully vaccinated and would like to remove your mask while inside The Glitter Lab.

I am fully vaccinated against COVID 19 according to the CDC guidelines and willing to show proof of such if asked.

If I am experiencing any symptoms of any illness other than COVID 19 (ie sneezing, coughing, runny nose, sore throat etc assocaited with the common cold, flu or other contagious illness) I will wear a mask during my appointment.

I understand that my technician is fully vaccinated against COVID 19 according to CDC guidelines and is willing to show proof of such if asked. My technician will also wear a mask if experiencing symptoms of any illness other than COVID 19.
